How to register

Before you register with us – please check our Practice Area Map to make sure you are in our practice area or check the practice covers your postcode here: Find a GP – NHS

You can either pick up a registration form from the practice or click here to register 

We require the information below to compete your registration:

  • Name and address of your last doctor
  • The address at which you were registered with your last doctor
  • If you are from overseas, your place of birth and the date you arrived in the UK
  • Your NHS number
  • Your signature

We aim to have your registration complete within 7 to 10 working days.

A patient questionnaire will also be provided which asks questions about your lifestyle and previous medical history. The practice will invite you for a health check with the practice nurse along with annual health checks at specific clinics should you suffer with certain illnesses.

We recommend all patients sign up to one of the following online services:

To set up access to online services at the same time as registering you will need to provide visual identification such as a passport or driving licence and one utility bill.

Every patient is now allocated a named GP. This may be different from the GP you usually see. This does not change the way you book appointments. It means that one GP will have overall responsibility for your care at the surgery. The practice will make reasonable efforts to accommodate any requests regarding any preferences to be with a specific GP. Should you wish to know your named GP please contact Reception.
