Sickness Certificates (Sick/Fit notes)
For any illness lasting seven days or less you do not require a doctor’s sickness certificate. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC Website.
If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay). We will be able to provide you with a sick note.
You can request a first time or continuation sick note by either completing an online request or calling the practice on 01642 607248.
GP Letter / Medical Certificates
For all other private administration requests such as GP letters, private medical certificates, insurance reports etc please submit an online request, this will be reviewed by the admin team who will then contact you regarding a price for completion of the work.
Alternatively, please contact the surgery via email nencicb-tv.amc@nhs.net or hand in your request to our reception team.
Please note the following information before submitting your request to the admin team:
- We require cash payments in advance for any none NHS work i.e letters, insurance reports.
- Requests may take up to 14 working days to complete.
- Prices may vary depending on the complexity of the report.
- It is up to the GPs to decide if they can complete private work, we are not obliged to accept your request.
